First Visit

The first visit with the doctor is 90 minutes long and includes a complete health history, consultation, examination, recommendations and initial treatment.

Please come prepared with the following:

1. A completed new-patient questionnaire

You may download it here, or we can send it to you via fax, email or US mail.

2. Copies of relevant medical records and/or lab results

You can arrange to have other doctors send them to Transformational Healthcare in advance or bring copies with you.


Follow-up Visits

Follow-up visits are 30 or 60 minutes. The doctor will make recommendations regarding the length and frequency of your visits. Follow-up visits consist of hands-on treatments, as well as supportive consults in which we review your progress, lab-test results and discuss any questions you may have regarding your health and well-being.

Insurance Reimbursement

On the day of your visit, you may pay by cash, personal check or credit card (Visa, MasterCard or American Express). We do not accept insurance or Medicare assignment for payment, nor are we a member of any insurance plan. We will provide you with detailed insurance forms, which you can submit to your carrier for reimbursement consideration. If you are covered by Medicare, we will submit your HCFA form on your behalf.

Most insurance companies in New York State are required to cover chiropractic services. If insurance company reimbursement is a priority for you, please speak with your insurance company or employer directly about your out-of-network insurance coverage and reimbursement possibilities. Transformational Healthcare's Nutrition Consultation

The first nutrition visit with Dr. Hilty is 30 minutes long and includes a consultation, functional body scan, and specific nutrition and life style recommendations.

The initial 30 minute nutrition consultation and follow-ups are for the following purposes:
  • To discuss dietary concerns, food choices and answer question related to eating and lifestyle habits.

  • To determine the optimal nutrition for your needs based on your history (symptoms, age, sex, etc.) and body function to enhance your overall health and well-being.

  • To be educated and empowered in developing long-term healthy habits that will improve and extend your life.
Follow-ups in most cases are monthly and are where feedback is shared and new recommendations and adjustments are made to your nutrition based on your current health state.

The 30 minute nutrition consult is not intended to:
  • Treat serious chronic illness- if you are experiencing significant challenges with your health you need to schedule a regular office visit with Dr. Hilty, or she would be happy to help you find another healthcare provider that suits your needs.

Dr. Hilty offers a complimentary 15-minute phone consult wherein you can discuss your health goals and determine which visit is the best fit for you.

Nutrition Visit Fees
Initial Consultation: $100- 30 minute visit
Monthly follow-ups: $65- 15 minute visit
Dr. Hilty is available by email or phone between your follow-up visits to answer question and provide support to help you achieve optimal results.

Insurance Reimbursement
This visit is not covered by health insurance due to insurance providers not covering wellness/prevention health care. To find out if your flex-spending can be applied to this visit and supplements, please inquire directly to your HR person or insurance company.


Cancellations and Missed Appointments

We understand that your schedule can change. We ask you to understand that we have reserved our time, expecting you to keep the appointment. If you must cancel, please give us at least 24 hours' notice, so that we may accommodate others. If you are running late, we may not be able to extend your appointment, as that will take away time from the patient after you. Missed appointments will be charged the full session fee.
